CANCELLATION POLICY

Appointment times represent time set aside by Dr. Koffler and Dr. Karpecki to address your
specific medical and vision needs. The care of our patients is the primary goal of Koffler
Vision Group. Due to an increase in patient cancellations and no shows for same day
appointments, we are having difficulty seeing the patients that need our care.

Therefore, our office is regrettably instituting a new policy. A charge of $35.00 will be
billed to the patient when a scheduled appointment is not cancelled 24 hours in advance.
This fee is not covered by your insurance company and payment is the responsibility of
the patient.

PLEASE BE COURTEOUS ENOUGH TO CANCEL YOUR APPOINTMENT 48
HOURS IN ADVANCE TO ALLOW ADEQUATE TIME FOR US TO RESCHEDULE
ANOTHER PATIENT IN YOUR TIME SLOT.
